Home and Community Care Program for Younger People

The Home and Community Care Program for Younger People (HACC PYP) provides support services for people with disabilities and their carers.

About the program

Mount Alexander Shire Council delivers the Home and Community Care Program for Younger People, which is funded by the Victorian Government. The program provides support with daily tasks for individuals who are 65 years and under (or 50 years and under for Aboriginal and Torres Strait Islander people) to assist you to remain living independently at home and in the community.

Types of services

The main types of services are:

  • Domestic Assistance
  • Shopping
  • Personal Care
  • Access and Support Program
  • Referral to allied health services (such as physiotherapy, podiatry and dietetics)
  • Delivered Meals and Community Based Lunches
  • Social Support Groups
  • Home Safety and Property Maintenance
  • Respite Services
  • Friendly Visiting Services
  • Transport Services provided by volunteers

How to access our services

To access services funded by the Home and Community Care Program for Younger People (HACC-PYP), you must undergo a living at home assessment to determine if you are eligible.

If you require urgent hygiene or meals support, please contact the Community Wellbeing team on 5471 1766

If you or someone you are concerned about requires extra support to understand or accept services or to contact My Aged Care you can contact the Access and Support Officer on 5471 1735 who will assist as required.

One of our Assessment Officers will visit you in your home to discuss how you are managing with everyday tasks and any difficulties you may be experiencing. The Assessment Officer may ask some questions in order to gain a clear understanding of your current situation and how best to help you. This will help to identify any support services that may be appropriate to meet your needs.

Client fee contribution

Fees apply to all of our services. Community Wellbeing HACC-PYP fees are subsidised by the State Government.

You are required to pay a small contribution for the support you receive, if you can afford to.

We provided you with a schedule of fees at your Assessment visit. Your fees are determined at this visit.

Fees are based on your income and individual circumstances.

We recognise that some people may have difficulties contributing to the cost of their services. In cases of identified financial hardship, your fees may be reduced or waived.

Please contact the office on 5471 1766 if your circumstances change and you wish to discuss your fees.

We review our client contribution fees annually and we advise you in writing of any changes to these. Your accounts is issued every four weeks and your account statement will be sent to you at your nominated postal address. You can pay your account using the options explained on the bottom of your account.
