Parents or guardians must sign a consent card before any vaccine can be given.
On the day of vaccination, students will bring home a form stating which vaccines they received. This form should be kept with previous immunisation records.
If your child was absent on the day of vaccinations please contact a Public and Environmental Health Officer for advice on 03 5471 1700.
We can provide immunisation records for people immunised through a Council service.
The fee for this record is $39.60.

We cannot provide records of immunisations given by doctors or other providers.
Once your child turns 14, you’ll no longer be able to access their immunisation history statement. They’ll be able to view it using their Medicare online account through MyGov. They can also use the Express Plus Medicare mobile app.
