The role

Key responsibilities undertaken by the HACC-PYP and Aged Care Intake Officer include:

  • Undertake HACC-PYP and Aged Care intake assessments and reviews of eligible clients, to support the development of goal directed individual support plans promoting a strengths-based approach.
  • Undertake the intake function, assessing incoming referrals within a set time frame.
  • Operate under the principles of client first solution focused.
  • Develop and foster strong client and service relationships and advocate on behalf of clients and their families where required.
  • Provide backfill support to Intake Assessment Team by undertaking commencement visits and service reviews for the Commonwealth Home Support Program (CHSP).
  • Provide responsive and supportive customer service to clients and other industry partners.
  • Promote and develop a culture within the team that reflects the vision and values of the organisation.

Our ideal candidate

To be successful in this role you will have:

  • Experience working in a Health or Community Care environment that supports independent living and participation in the community.
  • Clinical and or allied health qualifications highly desirable.
  • Knowledge and experience in supporting frail aged people and people with a disability and their carers / families to live independently to remain in their home.
  • Knowledge and experience with both Home and Community Care Program for Younger People (HACC-PYP) and Commonwealth Home Support Program (CHSP).
  • Ability to use initiative, liaise with stakeholders, work with minimal supervision and work collaboratively within a team environment.
  • Current Driver’s licence.
  • Working with Children Check.
  • Current First Aid Level 2.
  • Competent skills in Microsoft Word, Excel, and the ability to adapt to new systems and processes; experience with Carelink+ is desirable.
